Botox Membership Terms and Conditions

Membership Fee:

• Membership requires a monthly fee of $99/month for Jream Tox (up to 36 units per treatment) and $145/month for Jream Tax Plus (up to 50 units per treatment), charged automatically on the sign-up date of each month.

• A one-time sign-up fee of $99 is required which includes account set up, guarantees member-only pricing at $9/unit, waived fee after hour/VIP appointments twice per year, special perks such as discounts on fillers, weight loss programs, and skincare products.
   

Membership Benefits:

• Members are eligible for Botox at a discounted rate of $9/unit for additional units outside of membership allotments.

• Additional benefits include waived fee after hour/VIP appointments twice per year, special perks such as discounts on fillers, weight loss programs, and skincare products.

 

Usage of Units:

• Units allotted in the membership are to be used during the treatment visit and for the any follow up visits needed. An additional 4 units follow-up units are complimentary to fix any asymmetries from the initial treatment. Any additional units needed will be charged at $9/unit 

• Units expire at 3 month intervals.
   

Cancellation Policy:

• Introductory period: Members must remain enrolled for a minimum of 3 months before cancelling

• Early termination within the first 3 months will incur a cancellation fee of $250.

Refund Policy:

• Membership fees are non-refundable, even if benefits are not used.
   

Appointments and Scheduling:

• Membership benefits apply only to appointments scheduled and completed while the membership is active.

• Members are responsible for booking their appointments online at jreamaestheticsllc.janeapp.com. Please text 636-673-4442 for after hours/VIP appointments.

• A no-show or late cancellation fee of $50 will apply if appointments are canceled within 24 hours.

 

Eligibility:

• Botox treatments are subject to consultation and approval by a qualified provider.

• Jream Aesthetics reserves the right to deny treatment based on medical considerations.

Transferability:

• Membership benefits are non-transferable and apply solely to the enrolled member.
   

Changes to Membership Terms:

• Jream Aesthetics reserves the right to modify membership terms, fees, or benefits with 30 days written notice.
   

Termination by Jream Aesthetics:

• Jream Aesthetics may terminate the membership at its discretion for reasons including but not limited to misuse of benefits, non-payment, or inappropriate behavior.
   

Payment Authorization:

• By enrolling, members authorize Jream Aesthetics to charge the payment method on file for recurring membership fees.
